Is there one way to answer this question/problem that's like: don't do something that requires changing the culture or organization, focus on improving the tools and/or introducing new tools that incrementally improve how things are currently done. Put another way, what if we assume the organization (the communication paths) are as good as they can get, given whatever constraints and needs exist, and what we need to do is fix what we have because we can't change it. Another way: how could you fix your marriage without getting divorced? (I mean, of course, well, not YOUR marriage, nor mine - just a metaphorical "you.")
